Metolar XR 25 mg has been formulated to provide a controlled and predictable release of metoprolol for once-daily administration. Metolar is indicated for the treatment of hypertension (high blood pressure). It may be used alone or along with antihypertensive medication. It is used to treat angina pectoris (chest pain). This medicine is also effective at treating stable, symptomatic heart failure. Metolar extended-release tablet contains Metoprolol succinate, which causes blood vessels to relax, resulting in easy blood flow.
Metolar extended-release tablets should not be used in patients with known hypersensitivity to metoprolol succinate and other medical components. It should not be used in patients with severe bradycardia, sick sinus syndrome without a pacemaker, and greater than one-degree heart block. Also, avoid taking the medication in case of cardiogenic shock or decompensated heart failure. Abrupt cessation may worsen myocardial ischemia (when your heart muscles are not getting enough blood). Worsening cardiac failure may occur. Metolar XR should be used with caution in patients with impaired hepatic insufficiency.
Most side effects of Metoprolol XR tablets have been mild and transient. Common side effects of Metolar extended-release tablets include dizziness, tiredness, shortness of breath, diarrhoea, pruritus, depression, bradycardia, and rash.
Metolar extended-release tablets are intended for once-daily administration. The dose should be individualized to treat hypertension and angina; titration may be needed in some patients.
— For hypertension, the usual initial dosage is 25 to 100 mg daily in a single dose, whether used alone or combined with a diuretic. The dosage may be increased weekly until optimum blood pressure reduction is achieved. Generally, the maximum effect of any given dosage level will be apparent after one week of therapy. A dosage over 400 mg should not be exceeded.
— Angina pectoris - The dosage of metolar XR extended-release tablets should be individualized. The usual initial dosage is 100 mg once daily, given in a single dose. The dosage may gradually increase weekly until an optimum clinical response has been obtained or the heart rate slows. A dosage above 400 mg has not been studied. IF therapy needs to be discontinued, the dosage should be reduced gradually over 1 to 2 weeks.
— For heart failure - Dosage must be individualized and closely monitored during the treatment. Before starting treatment with a metoprolol extended-release tablet, the dosing of diuretics, ACE inhibitors, and digitalis (if instructed to use) should be stabilized. The recommended dose is 25 mg once daily for two weeks. The dose should not be increased until symptoms of worsening heart failure have stabilized. If heart failure patients experience bradycardia, the dose of Metolar XR should be reduced.
In case of hepatic insufficiency, consider initiating metolar XR at low doses and gradually increasing dosage to optimize therapy while monitoring closely for side effects.
